Cardiologist: their job is to evaluate your heart health and make recommendations for future monitoring and treatments. Their goal is to optimize your heart’s function and they will work with you to develop an individualized plan of care for your heart.
Nursing: the nurses in the VPACH clinic manage the care coordination and education needs for the outpatient VPACH community. They provide education about your heart condition and lifestyle choices, respond to phone calls or emails from patients who need health support, and help to coordinate some aspects of care – like surgery and procedures.
Clinical Nurse Specialist: supports patient and program needs with complex care planning, providing nursing support, policy development, quality improvement and research. If you are admitted to hospital or require complex care planning you may be contacted.
Social Work: provide supportive social services and can help you with travel, accommodation, disability benefits and connecting you with the health services you require.
Genetic Counsellor: provide information about how genetic conditions might affect you or your family. They also provide supportive counseling about the testing process for genetic conditions.
Pharmacist: responsible for the safe and appropriate use of medications for people in the VPACH Clinic.
They also provide medication education and can help with applying for special authority coverage.
Clerical Team: these members of the VPACH team will support you to secure an appointment and all the testing and imaging required for that. They have a huge role in supporting all the processes that make the VPACH clinic a success.

Partnering with those we serve, and their families — however they define their families — in their own care is now a well-accepted path to the best experience and outcomes. We believe that through building trust and respect, the relationships we establish with one another have the potential to dismantle the power imbalances that have traditionally existed between caregivers and patients. And that is better care. We believe that by partnering with those we serve, we can offer care that is valuable and valued by them. And what matters to them, is what matters most.
